I did it once (the first week it came out on normal) and I just started again a hour ago on Evil difficulty .

What's the difference? It doesn't seem harder than on normal or I forgot.

There's not much of a difference - enemies have more HP, and you can get weapons from regular monsters, but it's pretty similar to Normal.
